Migration Request Form

Please fill out the following form to kick off the process for migrating your website from the Fulton Schools Divi Theme to Pitchfork.

Please include the full name, ASURITE and email address for the content owner of the site.

The following fields will be used to construct the primary user account for the new website, This should be the faculty or staff member who “owns” the website, not a student or other affiliate with the lab group. Additional users can be added to the web site using the following guide once the site has been built.

This is the user name that you use to sign into various ASU services including my.asu.edu

Email address(Required)

Your email address will be used to communicate with you about future updates to the website platform.

Select your current website URL from the list below.(Required)

Select the domain name for your website(Required)

labs.engineering.asu.edu/

Provide a unique folder name to complete the URL.(Required)

The majority of ASU faculty members are using something like {lastname} or {lab-name} to uniquely identify their site.

Will you need help migrating your website to its new location?(Required)
